Privacy Policy
This Privacy Policy explains how The Health Collaborative collects, uses, shares, and protects information submitted through this website and related online services.
Last Updated: September 15, 2026
Scope and audience
This website is intended for users located in the United States. By using this website, submitting a form, registering for an event, or interacting with our online services, you acknowledge the practices described in this Privacy Policy.
This website is not intended to knowingly collect personal information from children under 13. Users under 18 should use this website only with the permission and involvement of a parent or legal guardian.
Information we collect from public website users
You may browse many parts of the website without signing in. When you choose to submit information, we may collect the information you provide, including:
-
Contact form details, such as name, email address, phone number, and message content.
-
Event registration details, such as first name, last name, email address, phone number, organization, title or position, registration type, and Community Health Worker status.
-
Community Health Worker registration or certification number if you provide one.
-
Public blog comment information, such as display name and comment content, if commenting is available.
-
Download or interaction records, such as the report or file requested and the status of that request.
Information from authorized users
Account creation is controlled by The Health Collaborative. Visitors cannot create their own accounts. Authorized users, such as administrators or content managers, may sign in to manage website content, registrations, reports, events, news, blog posts, banner settings, or other administrative features.
For authorized users, we may process account identifiers, authentication status, user group or role information, and records of administrative actions needed to operate and protect the website.
Payment and registration processing
Registration payments may be processed through Zeffy or another third-party payment service. When you continue to payment, you may be redirected to a payment page that collects payment information, including full name, email address, and payment method details.
The Health Collaborative does not store full payment card numbers or bank account details on this website. We may store limited payment confirmation information needed to manage registration status, such as whether a payment is pending or completed, the registration reference, and limited payment reconciliation notes. We do not intend to store full receipt details or unnecessary payment metadata.
Zeffy and other payment providers process payment information according to their own terms and privacy practices. You should review the applicable payment provider policies when using their services.
Health and sensitive information
This website is not intended to collect medical records, diagnoses, insurance identification numbers, treatment information, or other sensitive health information. Please do not submit sensitive health information through contact forms, registration forms, blog comments, or message fields.
How we use information
We may use information collected through the website to:
-
Respond to inquiries and requests for information.
-
Process event registrations and manage attendance records.
-
Coordinate registration payment reconciliation.
-
Send transactional emails, confirmations, operational notices, or responses to submitted messages.
-
Operate, secure, troubleshoot, and improve the website and related systems.
-
Maintain internal records, reporting, program documentation, and audit trails.
-
Comply with legal, contractual, grant, accounting, or administrative obligations.
We do not use submitted email addresses for future marketing or event communications unless a separate communication process or consent applies.
Analytics, cookies, and similar technologies
Our site uses browser storage for essential operation and optional features. Optional features are off until you choose to enable them. We have not installed Google Analytics or Meta Pixel; enabled YouTube embeds may use their own advertising technologies.
Change cookie preferences
-
Essential: AWS Amplify/Cognito storage supports secure sign-in and access to site content. Our hc-cookie-consent-v1 local-storage entry remembers your privacy choice for 180 days. These entries are not controlled by optional switches.
-
Remembered preferences: healthbridge-favorites-v1 stores resource favorites on this device until removed. healthbridge-search-state restores directory searches, including entered search terms and guided-search answers, for the current browser tab. Without this permission, favorites and searches remain only in page memory until reload.
-
Image caching: Local-storage entries beginning with s3image: cache image links valid for one hour. Disabling this category removes these entries; images continue to load.
-
External content: YouTube videos, Jawg map tiles, Mapbox address suggestions, and address lookups through OpenStreetMap’s Nominatim service load only when this category is enabled. These providers receive connection information such as your IP address and may use their own storage. YouTube can use cookies for preferences, security, measurement, and advertising.
In our review of the previous YouTube embeds, cookies included YSC, VISITOR_INFO1_LIVE, VISITOR_PRIVACY_METADATA, __Secure-YNID, and __Secure-ROLLOUT_TOKEN. Provider cookie names and lifetimes can vary. Embedded videos now use YouTube’s privacy-enhanced domain and still require your external-content permission.
Use “Cookie preferences” in the footer to change or withdraw permission at any time. Turning off a storage category deletes its saved entries. Turning off external content unloads embeds and stops future requests from those features. We cannot delete cookies on another provider’s domain; use your browser settings to remove them. Links that open other websites are governed by those websites’ own privacy choices.
Choices apply to this browser. We ask again after 180 days or when your stored choice is removed or invalid. If browser storage is blocked, choices apply only for the current visit.
SMS communications
If you voluntarily opt in to receive SMS communications, The Health Collaborative may send recurring text messages to the mobile phone number you provide.
Informational SMS consent and marketing or promotional SMS consent are collected separately. Agreeing to receive informational messages does not authorize The Health Collaborative to send marketing or promotional messages.
Consent to receive text messages is voluntary and is not a condition of receiving services, assistance, or completing any transaction with The Health Collaborative.
Message frequency varies based on your service interaction.
Message and data rates may apply.
You may opt out at any time by replying STOP to any text message. After opting out, you will no longer receive SMS messages unless you opt in again.
Opting out of SMS messages will not affect your eligibility for or access to services from The Health Collaborative.
For assistance, reply HELP, call The Health Collaborative at (210) 481-2573, or email info@healthcollaborative.net.
For complete messaging terms, review our SMS Terms and Conditions.
SMS Privacy & Mobile Data Non-Sharing Disclosure
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties or affiliates for marketing purposes.

Service providers and infrastructure
We may use third-party service providers and infrastructure providers, including Amazon Web Services, Zeffy or other payment providers, email delivery providers, analytics providers, storage services, and website hosting tools. Information may be processed and stored in cloud systems used to operate the website.
Authorized administrators may upload or manage content, images, reports, files, events, news, blog posts, and related website materials through cloud-backed systems.
Retention
We keep information for as long as reasonably necessary to provide services, manage registrations, operate the website, support organizational records, comply with legal or grant obligations, resolve disputes, and maintain security. Retention periods may vary depending on the type of information and our operational needs.
Your choices and requests
You may request that we delete personal information you submitted through the website, subject to legal, accounting, security, payment reconciliation, grant, reporting, or operational recordkeeping needs. We do not currently offer a general self-service export or copy of all submitted data.
To make a privacy request, contact us using the information below. We may need to verify your identity before acting on a request.
Texas and other privacy rights
Depending on where you live and which privacy laws apply, you may have rights to request access, correction, deletion, or information about how certain personal information is used or shared. We will review privacy requests and respond as required by applicable law.
California and other state privacy laws may provide additional rights for certain residents. We do not sell personal information and do not use personal information for cross-context behavioral advertising.
Security
We use reasonable administrative, technical, and organizational safeguards to protect information. These safeguards may include access controls, account-based permissions for authorized users, cloud service protections, and operational security practices. No online system can be guaranteed to be completely secure.

The website may link to third-party websites, services, payment pages, resources, and community partners. We are not responsible for the privacy practices, content, security, or availability of third-party websites or services.
Changes to this policy
We may update this Privacy Policy from time to time. Updates will be posted on this page with a revised last updated date. Continued use of the website after an update means the updated policy applies going forward.
